Please note: All online registration forms must be completed in full to secure and finalize your appointment time.



To ensure your first visit is efficient and thorough, please prepare the following requirements:


Required Documents & Information

  • Government-Issued Photo ID: A valid driver’s license, passport, or other official state ID.
  • Current Insurance Card: A physical or printed copy of your active health insurance card is required for each visit.
  • Medication List: A comprehensive list of all current prescriptions, over-the-counter medications, vitamins, and herbal supplements, including dosages.
  • Co-payment: Any applicable co-pays or past due balances are due at the time of check-in.



Helpful Medical Records

  • Immunization History: Records of past vaccinations, including recent flu or COVID-19 shots.
  • Past Medical Records: Copies of recent lab results (within 6 months), specialist reports, or summaries of recent hospital/ER visits.
  • Family Health History: Information regarding major diagnoses for immediate family members (parents, siblings, grandparents).



Preparing for Your Visit

  • Arrive Early: We recommend arriving 15–20 minutes before your scheduled time to complete any remaining intake steps.
  • Note Your Symptoms: Write down a detailed list of current health concerns, including when they started and what makes them better or worse.
  • Questions for the Doctor: Prepare a short list of specific questions or goals you wish to discuss during your time with the provider.
